Do Tweet replies to official handles of Prime Ministers really reach the person?

It probably depends on the country, and on the prime minister. It's difficult to see how a busy prime minister will have time to look at every reply on his Twitter account, but I know that in some countries the PR person in charge of the account will bring highly relevant replies to the PM's notice.

Nice to know. I agree that PM directly or through PR may surely have read and replied and even taken action.

You can send him a message via his personal website. The selected messages are read and replied by the Prime Minister himself.

Probably not, especially not from the Official handles that everbody knows. Self-important people use phalanxes of flunkies to intercept and triage unsolicited communications. Self-important people receive thousands of tweets a day. If they answred or read them personally, they would not have any time to do what they are supposed to do.
