It uses the available detection devices in your MacBook to find out if you are still at the computer, and if you are, it tells the MacBook not to dim the screen and go to sleep because it knows you are still at the computer working.
Should I Sleep only activates a few seconds before your computer goes to sleep, so it does not use much battery. It uses your computer’s camera and microphone to check for faces, movement, and ambient noise. If it detects any of these, it knows the computer is still in use and it prevents it from going to sleep. It can also stop sleeping if there is an external display connected and the old fashioned way where you define a certain amount of time before it should go to sleep.
