Open a Finder window. Select MacintoshHD in the Sidebar on the left. Then open the Library folder, the Application Support folder, the InputManagers folder, and the Scripting Additions folder.

The files in the Application Support folder: SIMBL/Plugins/CT2285220.bundle
    Move those to the Trash

Move all the toolbar related files to the Trash, then relaunch Safari.

Sounds like a third party Safari add on that cannot be deleted by "customizing the toolbar" ..
Follow the instructions here > http://support.apple.com/kb/TS3230?viewlocale=en_US

or try this alternate : View>Customize Toolbar and delete it from there.
