The Indian Government recently banned several websites including Google Docs. I wanted to create a confession page on Facebook and previously used the form feature of Google Docs for that. Now I require another method to create forms for the confession pages. Can you suggest any?

Earlier,Google Docs was banned in India by mistake as being a part of websites for illegal streaming in World Cup 2014.But now,it's working fine as I'd checked it.
Are you sure about the ban on Google DOCs by Indian Government?
