VisitorContact makes it easy for anyone to create custom contact forms and buttons. The contact forms created with VisitorContact can show all possible ways for someone to get in touch with the person (Twitter ID, address, maps, phone number, email etc).

After the user has registered and customized his/her contact details, there are two different ways to add the form to their website.
