It has a simple AJAX based interface which asks you to type the future date, the message and the email address and then click send to receive it on that date.

Futurecovery pitches itself as a promise sending app which asks you to write down a promise you make to yourself, like accomplishing something by a particular date and then send that email to yourself in future to check if you kept up with that promise you made to yourself. However, this app can also be used to send reminders or other emails in future to yourself or your friends.
